For more than 20 years, HomeTeam has provided home-based primary care for older adults across the Greater Victoria area. Our interdisciplinary team includes physicians, nurses, physiotherapists, and a rehabilitation assistant who work closely together to support patients in their own homes.

About the Role

This is a community-based nursing position focused on seniors living in their own homes or retirement residences. Patients vary in their level of independence and medical complexity.

This role does not involve long-term care work. Our patients remain in the community, and care is provided where they live.

The structure of the practice allows nurses to focus on quality care rather than volume. The goal is to improve quality of life, support caregivers, and help patients remain safely at home whenever possible.

What You Will Do

  • Conduct detailed patient histories and assessments

  • Monitor and manage acute and chronic conditions

  • Perform clinical procedures such as wound care, injections, suture removal, and ear syringing

  • Administer vaccinations and medications

  • Monitor nutrition, medications, and overall health status

  • Perform screening and preventive care

  • Collect specimens and complete point-of-care testing

  • Provide education to patients and families

  • Work closely with physicians and the interdisciplinary team

  • Coordinate care with community and hospital partners

Location and Logistics

  • Service area includes Victoria, Oak Bay, Gordon Head, Royal Oak, and Esquimalt

  • Mileage is reimbursed

  • Cell phone use is reimbursed

Compensation and Benefits

  • Competitive salary based on experience

Requirements

  • Registered Nurse licensed in British Columbia

  • Currently located in the Greater Victoria area

  • Clinical experience (community or geriatrics an asset)

  • Strong communication and teamwork skills

  • Patient-centered approach to care

Position Details

  • Full-time position, part-time considered

  • Start date May or June 2026, flexible
