Location: Ontario, CA

Overview:
Our organization in Ontario is seeking a full-time Registered Nurse to support care coordination and healthcare services for children within a collaborative, trauma-informed environment. This role offers the opportunity to work closely with healthcare providers and multidisciplinary teams while helping ensure high-quality, patient-centered care.

Position Offers:
• Full-time schedule, Monday–Friday

Key Responsibilities:
• Coordinate medical care and healthcare services for children in the program
• Serve as the primary liaison between healthcare providers, staff, and families
• Manage medical appointments, medications, immunizations, and ongoing healthcare needs
• Maintain accurate medical documentation and ensure compliance with healthcare regulations
• Monitor client health outcomes and support continuity of care planning
• Collaborate with multidisciplinary teams on care coordination and discharge planning
• Provide guidance to staff regarding medical protocols and procedures
• Maintain a safe, supportive, and trauma-informed environment for clients

Qualifications:
• Active California Registered Nurse license
• 2+ years of child welfare experience
• 1+ year of experience in public health, care coordination, or clinical care
• Strong communication, organizational, and documentation skills
• Ability to work effectively in a team environment
