Responsibilities:

  • Conduct comprehensive health assessments, including medical histories and physical examinations in collaboration with cardiologists and Internal Medicine specialists.
  • Provide patient education and counseling regarding disease prevention, treatment options, and healthy lifestyle choices.
  • Monitor patient progress and adjust treatment plans as required.
  • Collaborate with physicians, specialists, allied health professionals, and community resources to ensure continuity of care.
  • Maintain accurate and timely clinical documentation within the electronic medical record (EMR) system.
  • Participate in quality improvement initiatives and support best practices in patient care.
  • Adhere to all regulatory, professional, and organizational standard
  • Perform consultations with patients as assigned.
  • Perform stress test supervision as assigned.
  • Perform all medical tasks delegated by the attending physician.
  • Contribute to ensuring there is a safe environment for patients, staff and visitors.
  • Maintain safe and clean working environment by complying with procedures, rules, and regulations.
  • Call for assistance from health care support personnel, if required.
  • Document patient care services by charting in patient records.
  • Take all patient vitals and history.
  • Maintain patient information electronically.
  • Document and record medicine and prescriptions.
  • Perform any and all other tasks as assigned by the supervisor.

Qualifications, Experience & Requirements:

  • 0-2 years of related experience.
  • Nurse Practitioner certification is required. 
  • Member in good standing with the College of Nurses of Ontario.
  • Demonstrated knowledge and understanding of general medicine.
  • Demonstrated ability to collaborate and communicate effectively with health professionals and patients.
  • Must possess the ability to function effectively in a clinical setting.
  • Familiarity with electronic medical records (EMR).
  •  Knowledge of and demonstrated ability in corporate core competencies including customer service, communication, teamwork, initiative/self-management, accountability, dependability, flexibility and adaptability.
  • Computer literacy utilizing MS Office software applications and ability to research on the Internet.
  • Ability to work within a diverse community and to deal empathetically with patients, family members, etc., to contribute to their well-being.
  • Ability to work effectively independently or as a part of a multidisciplinary healthcare team.
  • Demonstrated commitment to continuing education and training.
  • Demonstrate effective decision-making and problem solving skills.
