What the Program Entails
SWCHC is well known for its excellent Community Health Centre (CHC) model and its professional health services team, but it has also become a leader in offering a wide range of community-building activities and advocacy programs. Our team of doctors, nurses, dietitians and more offer a range of medical services.
What You’ll Do
Reporting to the Director of Integrated Health Services, the Nurse Practitioner (NP) is a key member of the interdisciplinary team, delivering comprehensive, client-centred primary health care services within the Primary Health Care (PHC) clinic. The NP works to their full scope of practice in the same-day clinic and may also support a roster of clients. The NP provides culturally responsive care both independently and in collaboration with an interprofessional team. The successful candidate will demonstrate the ability to work autonomously and as part of a team, with a strong commitment to culturally safe, equitable, and client-centred care. This position requires availability to provide service one evening per week, until 8:00 p.m.
The NP is responsible for delivering primary health care services in accordance with the Standards of Practice for Registered Nurses in the Extended Class and the Expanded Nursing Services for Patients Act. This includes health promotion, counselling, disease prevention, diagnosis, treatment, and referrals to specialists or diagnostic testing. Providing clinical services, requiring the NP to manage a diverse range of health conditions while addressing the complex needs of the populations served.
The NP plays an active role in the development, implementation, monitoring, and evaluation of programs and services, ensuring that individuals with diverse health, social, cultural, and economic needs receive accessible and high-quality care. Functioning both autonomously and within a collaborative team environment, the NP contributes to advancing program goals, thereby enhancing health outcomes within the community.
