Summary of Position:

The Substance Use Nurse Practitioner (NP) works collaboratively with nurses, physicians, shelter case managers, community workers, and agency staff to deliver substance‑use care and limited primary care to clients experiencing homelessness, in alignment with PHIPA and client consent requirements.

The NP provides comprehensive, advanced assessments supported by the mobile substance‑use nurse to address both urgent and ongoing substance‑use needs, escalating concerns to a physician or Emergency Services when appropriate.

Working closely with shelter and city partners, the NP also offers social and emotional support, including crisis mitigation and case management. The role receives indirect supervision and ongoing guidance from a Nurse Manager.
