New Graduate Nurse Practitioners Welcome!

What if your next role gave you the opportunity to do more than treat patients; what if you could truly change their everyday lives?

Imagine being able to follow a resident's journey, build meaningful relationships with the people you care for, and see firsthand the difference your clinical expertise makes.

We're partnering withΒ a long-term care organization in Toronto, to find a passionate and dedicated Nurse Practitioner (NP) who wants to make a lasting impact on the lives of older adults.

Whether you're an experienced NP looking for a rewarding new chapter or a newly graduated Nurse Practitioner ready to begin your career, we'd love to connect with you!

Why This Opportunity Is Worth Exploring

In long-term care, you're not simply treating a condition. You're getting to know the person behind it.

You'll have the opportunity to:

  • Make a lasting impact. Build ongoing relationships with residents and families while playing a meaningful role in their health, comfort, and quality of life.

  • Practice to your full NP scope. Apply your advanced clinical knowledge to assess, diagnose, prescribe, and manage complex healthcare needs.

  • Be part of something bigger. Collaborate with an interdisciplinary healthcare team that shares your commitment to exceptional resident care.

  • Make a difference through proactive care. Help prevent avoidable hospital transfers, manage chronic conditions, and support residents in receiving timely care where they live.

  • Grow as a clinical leader. Share your expertise, contribute to clinical best practices, and help shape the delivery of resident-centered care.

  • Explore a rewarding career path. New graduate NPs are encouraged to apply, alongside experienced practitioners.

What You'll Be Doing

As a Nurse Practitioner, you'll play an important role in supporting the health and well-being of residents.

Your responsibilities will include:

  • Providing comprehensive assessments, diagnoses, treatment plans, and ongoing primary care.

  • Managing complex and chronic conditions, including diabetes, COPD, hypertension, and dementia.

  • Prescribing medications and treatments within your authorized scope of practice.

  • Collaborating with physicians, nursing teams, pharmacists, and other healthcare professionals.

  • Supporting residents and families through care planning, education, and important healthcare decisions.

  • Responding to changes in resident health and helping reduce unnecessary hospital visits.

  • Contributing to clinical education, quality improvement, and evidence-based care.

What We're Looking For

We're looking for someone who brings clinical curiosity, compassion, sound judgment, and a genuine interest in supporting older adults.

Requirements:

  • Registration as a Nurse Practitioner (RN Extended Class) with the College of Nurses of Ontario (CNO).

  • Strong clinical assessment, communication, and decision-making skills.

  • Current CPR certification and ability to obtain a satisfactory vulnerable sector check.

  • A collaborative, resident-centered approach to care.

Experience in geriatrics, primary care, or long-term care is an asset, but new graduate Nurse Practitioners are welcome and encouraged to apply!

Ready for a Role Where Your Work Truly Matters?

Some of the most rewarding moments in healthcare aren't the big ones.

They're the moments when you recognize a change in a resident before it becomes an emergency. When you reassure a worried family. When your clinical judgment helps someone remain comfortable in the place they call home.

If you're ready to explore an opportunity where your clinical skills, compassion, and commitment to care can make a meaningful difference, we'd love to hear from you.

Apply today or connect with our recruitment team for a confidential conversation.
