Gattuso Centre for Social Medicine and Population Health
The Gattuso Centre for Social Medicine seeks to address health inequities, working alongside community organizations, UHN clinicians, marginalized populations, persons with lived experience, patients, and all levels of policymaking to co-design, evaluate, and scale innovative data-driven programs that lead to better health. Applying a social determinants of health approach, the Social Medicine and Population Health Program aims to bridge health services and the broader social environment of UHN’s most medically and socially complex patients with the highest needs through the design, implementation, research and evaluation of new care models and initiatives to address the health implications of inequity.
Position Summary
The University Health Network is committed to high-quality and equitable care for all patients. UHN recognizes the significant health disparities experienced by patients with unmet health and social needs and the importance of providing care outside of traditional hospital settings — including in the community and at home. In response, UHN has established the Gattuso Centre for Social Medicine to transform how care is delivered to marginalized populations.
UHN is seeking Registered Nurses (RN) with specialized training in wound care to join our Social Medicine Program. This role will focus on addressing the social determinants of health for patients with complex medical and social needs, including those who frequently use hospital services and face barriers to care due to poverty, aging, being under-or-unhoused, mental health challenges, and substance use. Working as part of a broader interdisciplinary team, the RN will provide high-quality, compassionate care directly in community-based settings at 750 Dundas St. W., Dunn House (90 Dunn Ave), and the Social Medicine Mobile Health Clinic Vans.
The Registered Nurse (RN) with expertise in caring for patients with complex medical conditions, knowledge in chronic wound care, diabetic foot ulcer management and focused clinical assessments, will collaborate with the Social Medicine Nurse Practitioner, Community Health Workers, Peer Support Workers, UHN care teams, and community organizations to deliver integrated, person-centred care that meets patients where they are. The successful candidate will play an active role in shaping the scope and direction of nursing practice within the Social Medicine Program, offering a rare opportunity to help define how care is delivered in this innovative space.
