Position Summary:

  • Responsible for development of nursing care plans and counselling for patients and families dealing with paediatric diabetes

  • Responsible and accountable for decisions and actions in the provision and management of nursing care for patients with diabetes in a collaborative inter-professional team environment

  • Broad understanding of resources available in the community to help support caseload of patients through critical thinking and problem solving to ensure best practices are maintained

  • Assist and support clients to gain the knowledge, skills, attitude and self-awareness necessary to effectively manage his/her diabetes

  • Function as a part of a multidisciplinary team, and act as a resource for other health care providers and the community

  • Using a variety of teaching methods, incorporating the principles of adult education and motivational interviewing within a case management model

  • Develops appropriate education materials to meet the needs of the clients

 

Key Qualifications:

  • Registered in good standing with the College of Nurses of Ontario (CNO) or currently meet the eligibility criteria to register in good standing in the Temporary Class or General Class with the CNO

  • Certified Diabetes Educator

  • 3-5 years current paediatrics nursing experience; familiar with childhood diseases and treatments

  • Membership in the Diabetes Educator Section of the Canadian Diabetes Association preferred

  • Minimum 5 years' experience in counselling clients with diabetes on an individual and group basis

  • Good working knowledge of diabetes through the lifecycle and adult teaching principles

  • Highly effective decision making, problem solving and organizational skills combined with excellent interpersonal and communication skills, both oral and written

  • Proficiency in computer, experience with electronic medical record and good keyboarding skills are essential

  • Reliable, flexible, creative professional, able to work effectively and collaboratively in a multidisciplinary team approach required

  • Experience using medical directives an asset

  • Additional language an asset

  • Positive attendance and work record essential

  • Current member in good standing with College of Nurses

  • Current CPR

  • PALS preferred
