Position Summary:

As a Diabetes Educator, you will play a key role in supporting patients in the management of their diabetes journey and prevention its associated complications. Working within a collaborative, interprofessional team environment, you will provide education, clinical care, and support to empower patients to effectively manage their health.

 

Key Responsibilities:

  • Develop and implement individualized nursing care plans, providing education and counselling to patients with diabetes and related comorbidities (e.g., renal disease, cardiovascular disease, mental health conditions)

  • Assume accountability for clinical decision-making in the provision and coordination of patient care

  • Utilize critical thinking to connect patients with appropriate community resources and ensure evidence-based best practices are maintained

  • Serve as a clinical resource to the team for diabetes-related complications, including foot ulcers and other wound or skin concerns

  • Empower patients to build the knowledge, skills, and confidence required for effective self-management

  • Provide inpatient coverage at Trillium Health Partners, including bedside education and discharge planning support

  • Collaborate with multidisciplinary team members and act as a resource for healthcare providers and community partners

  • Apply a variety of teaching methods grounded in adult education principles and behavior change models (e.g., motivational interviewing, stages of change)

  • Facilitate group education sessions as required

  • Develop and adapt educational materials based on patient needs

  • Contribute to program planning, quality improvement, and evaluation initiatives

 

Key Qualifications:           

  • Minimum of three (3) years’ experience providing individual and group counselling to patients with diabetes
  • Certified Diabetes Educator (CDE) designation is required 
  • Membership in the Diabetes Educator Section of Diabetes Canada is considered an asset 
  • Strong knowledge of diabetes and associated comorbidities across the lifespan, including adult learning principles and patient engagement strategies 
  • Sound understanding of diabetes medications, including insulin management 
  • Experience and demonstrated interest in wound care, including management of diabetic foot ulcers (e.g., debridement, offloading devices); certification in or enrollment with NSWOCC, AWCCP, or equivalent programs is considered an asset 
  • Excellent clinical judgment, decision-making, problem-solving, and organizational skills 
  • Strong interpersonal and communication skills (both written and verbal) 
  • Proficiency with Microsoft Office and electronic medical records (EPIC), with strong keyboarding skills 
  • Ability to work independently and collaboratively within a multidisciplinary team 
  • Experience working with medical directives is considered an asset 
  • Fluency in an additional language is considered an asset 
  • Demonstrated reliability and a positive attendance record 
  • Member in good standing with the College of Nurses of Ontario
