Part-Time, 18.75 hours per week, Monday to Friday, days (with the possibility of some evenings).

This position is temporary until September 2028.

1. Current certification as a Registered Nurse from the College of Nurses of Ontario required 
2.     BScN an asset
3.     Canadian certification in Psychiatric and Mental Health Nursing CPMHN( C ) preferred
4.     BCLS certification
5.     Minimum of 5 years of recent clinical nursing experience (within the past 3 years) including experience in mental health, psychiatry, psychosocial oncology, or a related specialty.
6.     Recent experience working with patients and families affected by cancer is preferred

7. Recent experience working with the Adolescent and Young Adult (AYA) oncology population, or demonstrated knowledge of the unique developmental, psychosocial, and supportive care needs of adolescents and young adults with cancer, is preferred.
8.     Demonstrated expertise in clinical assessment, management and counselling in a broad range of mental health care issues 
9.     Working knowledge of psychiatric and mental health disorders and related psychiatric/ psychosocial interventions and treatment models such as\: supportive psychotherapy, cognitive behavioral therapy, solution-focused therapy, counseling, and crisis intervention or management in an adult community setting to clients suffering from mental illnesses or psycho-social difficulties.
10.    Comprehensive knowledge and experience in administration and/or monitoring of psychotropic medications

The Registered Nurse (RN) in Psychosocial Oncology at the JCC provides specialized nursing care to patients and families living with cancer and experiencing psychosocial distress and underlying supportive care needs.  The RN practices as a member of the Psychosocial Oncology  team and practices within a shared, collaborative practice model to optimize patient outcomes, improve access to services and ensure coordinated, evidence-informed care across the continuum.

Assignments within the Psychosocial Oncology portfolio may include Mental Health and Psychiatry services, the Adolescent and Young Adult (AYA) oncology program, and other specialized psychosocial oncology initiatives based on program needs. Working collaboratively with psychiatrists, psychologists, social workers, oncologists, and other members of the interdisciplinary team, the RN provides comprehensive assessment, care coordination, supportive interventions, and ongoing follow-up for patients with psychosocial, psychiatric, and age-specific supportive care needs.

Specific responsibilities may include\: 

·  Conduct comprehensive psychosocial, psychiatric, and physical health assessments of patients referred to the program; develop, implement, and evaluate individualized care plans and provide ongoing follow-up as appropriate.

·  Provide nursing care and support to patients within the Mental Health and Psychiatry portfolio, including those experiencing psychosocial distress, underlying mental health concerns, or psychiatric illness in the context of a cancer diagnosis and treatment.

·  Provide specialized nursing care and care coordination for the Adolescent and Young Adult (AYA) oncology population, addressing the unique developmental, psychosocial, mental health, fertility, sexual health, survivorship, and supportive care needs of young adults with cancer.

·  Complete best possible medication reconciliation at consultation and follow-up visits and document care in the JHCC electronic health record (EPIC).

·  Deliver between-appointment case management, care coordination, patient navigation, and ongoing psychosocial support to patients and their families.

·  Provide crisis assessment and intervention for patients presenting with acute psychosocial distress or psychiatric concerns and facilitate timely access to appropriate services.

·  Coordinate psychiatric and psychosocial care across inpatient, outpatient, community, and primary care settings to ensure seamless transitions and continuity of care.

·  Triage incoming referrals, conduct pre-screening assessments, and prioritize patients based on clinical need.

·  Maintain accurate and timely documentation in the JHCC electronic health record (EPIC) in accordance with College of Nurses of Ontario (CNO) standards and manage electronic communications and in-basket messages for the Psychosocial Oncology team.

·  Serve as a key liaison for the Psychosocial Oncology program, collaborating with physicians, nursing colleagues, allied health professionals, and community partners to support coordinated, patient-centred care.

·  Participate in program development, quality improvement initiatives, and the implementation of evidence-informed practices that enhance psychosocial oncology care.

·  Manage a dynamic workload effectively in a fast-paced ambulatory oncology environment while maintaining high standards of patient care and professional practice
 

The Juravinski Hospital and Cancer Center (JCHH) serves more than 1.7 million people in Central West Ontario. The JHCC includes the Juravinski Cancer Center (JCC), which is an outpatient facility where more than 7000 new patients with cancer are seen each year. The JCC provides consultation and specialized assessment, treatment and follow-up care for patients with cancer and their families. Services include comprehensive programs in systemic and radiation therapy, surgical oncology, urgent care, and comprehensive supportive care to meet the full range and severity of psychosocial and mental health needs.  The Cancer Program includes an in-patient oncology ward, hematology and bone marrow transplant ward, and oncology day services.
