Part-Time Licensed Practical Nurse (LPN)

Role & Responsibilities:

• Oversee the care of home care clients, ensuring their needs are met with quality and compassion.
• Conduct in-home client assessments, monitor client progress, and collaborate with the care team to develop and implement person-centered care plans.
• Ensure timely submission of reports, while providing support to other team members.
• Provide support to the Health Care Workers and Nurses, guiding them to deliver exceptional care and supporting their professional development.
• Perform wound assessments, assist the nursing team in developing and implementing treatment plans.
• Provide follow-up wound care in client’s homes while adhering to best practice standards.
• Drive to various locations in the Metro Vancouver area.
• Be available 15-30 hours per week (Monday to Friday)

Qualifications:

• Current LPN registration with the BCCNM and up-to-date CPR certification
• Current and clear Criminal Record Check
• Valid BC driver’s license, and reliable transportation to drive to various locations in the Metro Vancouver area
• Experience in wound assessments, and wound care
• Have a strong understanding of antibiotics therapy and adherence to safety and infection protocols.
• Ability to establish and maintain effective working relationships with other healthcare professionals, clients’ family members, and relevant organizations to meet clients’ best interests and goals.
• Excellent technical skills to ensure accurate and timely completion of our client care documentation using our online systems.

Pay rate: $32.00 - $40.00/hour
