Position Summary

Nurse Practitioner, Telemetry
The Nurse Practitioner (NP) is a registered nurse in the extended class (EC), who demonstrates the competencies to autonomously diagnose order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures within their legislated scope of practice. The NP demonstrates the following competencies established by the Canadian Nurses Association Canadian Nurse Practitioner Core Competency Framework (May 2010): Clinical Practice, Collaboration, Consultation and Referral, Research, and Leadership. The NP will provide specialized and advanced nursing care for both the inpatient ward (Telemetry) and the outpatient Cardiology clinics, within an inter-professional team model. The NP provides leadership, and is responsible for the promotion and enhancement of cardiac care across the health care continuum. The NP integrates research into practice, to ensure best practice standards are maintained and incorporated into clinical initiatives. The Nurse Practitioner Contributes to the education and professional development of nursing and the inter-professional team, and to the wider professional community within their area of specialization. Initiates and leads best practice initiatives that promote improved patient outcomes in their specialty area and/or on a corporate basis. Creates, utilizes and/or disseminates new nursing knowledge. All roles are essential in providing a safe and healthy environment for patients, staff, physicians, volunteers and others by ensuring that health and safety policies are followed
This position supports the Hospital’s academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for staff, physicians, volunteers, learners, patients and families.

On a practical level, you will

•Performs comprehensive nursing/medical history, physical and psychosocial assessment of the patient’s status, and development and implementation of a plan of care to address identified needs of patients with cardiac problems in the Outpatient Cardiology Clinics and 6 West Telemetry
•Provides direct, specialized cardiac care and/or consultation to patients, families, health care professionals, and community partners in situations where advanced nursing expertise is required (e.g., complex patient situations with unpredictable outcomes).
•Collaborates with the Supportive Cardiology team in identifying goals of care and advance end of life care planning.
•Provides leadership in establishing and meeting goals related to nursing and clinical practice; accountable to align goals with the strategic direction of the program and the organization.
•Initiates and performs diagnostic and therapeutic procedures for identification and management of cardiac problems as authorized through medical directives.
•Educates health care team members in best practice standards in the management of patients who demonstrate symptoms of heart failure, acute coronary syndromes, etc.
•Develops, implements, and evaluates effectiveness of clinical service programs in collaboration with the Program Director and Director of Professional Practice.
•Engages in or conducts nursing research and participates in interprofessional research projects, as required.
•Engages in scholarly activities, such as presentations and publications.

Qualifications

•Masters in Nursing, required.
•Minimum of 3 - 5 years clinical nursing experience in Critical Care or Cardiology
•Completion of a Nurse Practitioner program, required
•Registered and in good standing with the College of Nurses of Ontario (CNO) in the Extended Class, required
• Current Basic Cardiac Life Support certificate, required
• Current Advanced Cardiac Life Support (ACLS), required
•Experience with pacemaker/ICD interrogation preferred
•Previous experience as a Nurse Practitioner in Cardiology, Telemetry, Critical Care or Medicine an asset
•Certificate in Hospital & Acute Care for Nurse Practitioners an asset
•Cross appointment with the Bloomberg Faculty of Nursing or other academic Nursing institution an asset
•Experience in developing and leading quality improvement initiatives an asset
•Bilingualism in French/English an asset
