Position Summary

The Nurse Practitioner (RN-EC) is a self-directed advance practice prepared nurse who functions in an expanded nursing role within a collaborative interprofessional practice model. The RN-EC demonstrates a comprehensive knowledge base in patient care across the continuum, exhibiting a high degree of clinical competence in caring for patients of all ages and all stages of life, within the emergency department setting. The RN-EC scope of practice incorporates the knowledge, skills and judgment legislated by the College of Nurses of Ontario and required by role. The RN-EC provides comprehensive patient care to appropriately manage illness along with health promotion and disease prevention.

The RN-EC is responsible for working collaboratively within the interprofessional team to develop, implement, and evaluate patient plans of care. The RN-EC uses advanced knowledge, skill, and judgment when providing care and works to identify solutions for admission diversion for patients seen in the emergency department. The RN-EC will demonstrate expertise in coordinating and expediting community resources to support patients and precent hospital admissions. The RN-EC is able to collaborate with community organizations such as home and community care and Hospice Prince Edward in order to facilitate the efficient transitions that support the patients and their caregivers in the community.

The RN-EC utilizes a holistic approach that is grounded in evidence with an emphasis on management of acute and episodic conditions as well as health promotion and illness/injury prevention. The RN-EC functions in an expanded role demonstrating a high level of autonomy and is responsible for: conducting comprehensive client assessments, generating diagnoses, ordering, and interpreting diagnostic tests, prescribing pharmaceuticals, and performing procedures within the RN-EC legislated scope of practice. Working in collaboration with the inter-professional team the RN-EC will ensure a timely response to a client's changing health status and needs and provide direct patient care focusing on recognizing, managing, and stabilizing patient’s ever changing medical conditions in the emergency department. The RN-EC elicits confidence in consulting and coordinating with hospitalists and specialists while advocating for patients’ ongoing care needs. The RN-EC demonstrates efficiency in documentation and completion of in-patient admission orders and documentation.

Required

• Current registration in the Extended Class (RN-EC) with the College of Nurses of Ontario in required area of practice.
• Current certification in ACLS or proof of enrolment in a recertification course. You will be asked to provide a copy of your valid certificate at the time of hire/transfer.
• Minimum of three to five years’ recent experience in an Emergency Department with demonstrated progressive expertise required, emergency NP experience considered an asset.
• Bachelor of Science in Nursing Degree (BScN).
• Master’s Degree in Nursing (MSN) preferred or Primary Health Care Nurse Practitioner Certificate with relevant experience.
• Demonstrated ability to work both autonomously and function effectively in an interdisciplinary setting.
• Demonstrated ability for independent action, initiative, problem-solving, organization & prioritization.
• Demonstrated leadership, critical thinking, interpersonal and communication skills.
• Demonstrated commitment to patient and family-centered care.

Duties

Areas of Responsibility:
The responsibilities of the NP in this role are program-specific and can include, but not be limited to, daily clinical management of a defined group of patients and collaboration with interprofessional team to promote quality patient care and patient safety.

Practice - (60 - 80 %)
Nurse Practitioner (NP) practice will include primary responsibility for directing patient care through appropriate use of health care resources, co-coordinating and liaising with the interprofessional team throughout the continuum of care.

The NP will:
1. Provide direct care to selected patients based on the needs of the identified population and the NP’s specialty knowledge and skills. Direct care will include clinical assessment, ordering and monitoring treatments, enhancing quality and continuity, demonstrating specialty skills and/or to taking action in emergency situations as required.
2. Determine patient health care needs (in consultation with physician when required) through advanced history taking, psychosocial assessment, risk assessments, physical examinations/interval assessments, and interpretation of diagnostic tests.
3. Promote understanding of medical diagnosis and related diagnostic tests, procedures, and treatment plan with patient and family.
4. In collaboration with the patient and family, develop/initiate/modify health care plans, determines and implements required diagnostic testing and initiates (adjust and/or discontinue) pharmacological and non-pharmacological therapies as defined by the College of Nurses of Ontario (Practice Standard – Nurse Practitioners, 2008) or using medical directives when indicated (care outside of legislated scope of practice).
5. Provide patient care by leveraging technology and utilizing innovative modalities including, but not limited to, telephone nursing practice and electronic communication when appropriate in accordance with Quinte Health policies, procedures, and standards of practice.
6. Initiate and/or participates with interventions for the purpose of stabilizing the patient in urgent and emergent situations.
7. Direct referrals to the appropriate health care professional agency or service.
8. Accurately document assessment, plans, interventions, and evaluations of patient and/or family responses in the patient record (paper and/or electronic) as per Quinte Health policies, procedures and standards of practice.
9. Provide consultation to nursing, medicine, and other members of the interprofessional team.
10. Demonstrate sound clinical reasoning and employ evidence-based clinical practice guidelines to guide screening, assessment, diagnosis, treatment, and transition of care planning.
11. Lead communication between patients and interprofessional team members in anticipating, discussing, and resolving moral, ethical, and legal issues relevant to patient care at the individual and organizational level.
12. Develop and implement innovative approaches for complex practice issues and outcomes evaluation.
13. Participate in the development, implementation, and evaluation of best practices within a rural community hospital setting and broader regional/provincial context.
14. Lead the development and integration of health teaching information for the patient/family/significant others.

Education - (10-15%)
In collaboration with the patient/family/significant other and interprofessional team the NP takes the initiative to identify educational opportunities and works with the patient and team to develop and implement teaching and learning strategies and resources.

The NP will:
1. Develop and deliver teaching programs with the patient/family/significant others that focus on health promotion, disease prevention and impact of illness. Teaching programs will be appropriate to the patient’s developmental level, learning needs, readiness to learn, and cultural values and beliefs.
2. Work collaboratively with members of the internal interprofessional team and external networks to advance professional practice within the program and community hospital setting. The NP will mentor other health professionals in applying the principles of evidence-based practice.
3. Advance the process of knowledge transfer for the interprofessional team by being an expert role model in the integration of best practices through regular case reviews with members of the clinical care team.
4. Engage in reflective practice to identify and participate in professional development opportunities that will enhance clinical specialty, educational and leadership knowledge, and skills.

Research (5 - 15%)
Supports development and exchange of knowledge.

The NP will:
1. Be an active participant in reviewing literature and conducting critical analysis of current research to facilitate practice decisions that are evidence based.
2. Contribute to knowledge exchange through presentations as opportunities arise.
3. Develop and/or utilize appropriate assessment instruments to measure clinical outcomes and/or other components of clinical programs.
4. Initiates and participates in research projects as appropriate.

Leadership & Program Development (5 – 15%)
Manages change to improve clinical practice & patient outcomes through the development and implementation of the NP role at Quinte Health.

The NP will:
1. Use effective strategies for influencing team behaviour to encourage adoption of new NP roles in the rural community hospital setting.
2. Provide leadership in promoting and sustaining effective interprofessional collaboration to enhance outcomes-focused patient care.
3. Develop medical directives to broaden areas of patient care management and provide care, in collaboration with team members, which is beyond the independent scope of practice for the NP(RN-EC).
4. Participate in the ongoing development of NP roles and job descriptions and identify strategies for role sustainability within the corporation.
5. Collaborates with internal professional practice networks to advance nursing practice and enhance patient outcomes.
6. Represent Quinte Health on external networks and committees specific to the specialty area and NP practice as requested/required.
