Job Description: This position works as part of an interdisciplinary primary health care team. Under the direction of the Primary Health Care Manager, the Nurse Practitioner will provide primary health care services to the community. The Nurse Practitioner will be responsible for assessment, planning, implementation and evaluation of care for clients in the community both independently and collaboratively with a physician, depending upon the complexity of client needs. This includes: health promotion and protection; first line treatment and emergency services; provision of supportive counselling and instruction in management of medical conditions; disease and injury prevention; initiating and conducting community needs assessments and participating in evaluative projects; and collaboration with and referral to other health care professionals.

Experience

  • One year experience in community needs assessment/development/health promotion
  • 3 years experience as an RN in primary care, public health or acute care

Education

  • Bachelor of Science in Nursing (BScN)

Licenses

  • Valid Class 5 driver's license

Other Education and Training

  • Certification in recognized Primary Care NP equivalent program is acceptable
  • Basic Cardiac Life Support Certificate (BCLS)

  • Knowledge of common medical conditions and treatments.

  • Knowledge of community programs/services
  • Knowledge in the principles of Population Health Promotion

  • Demonstrates ability to assess the community for primary care needs.
  • Ability to provide holistic care to individuals,families & the community
  • Demonstrates empathy, trust and respect in the provision of care
