We are seeking a Nurse Practitioner (Casual position)

What will be your role?

In this role you will work in a dynamic, fast-paced environment where you will achieve excellence in providing home care services to VHA clients. In addition, you will act as a subject-matter expert for new initiatives and projects related to home care, community clinic, digital health, and in fostering vendor partnerships. Reporting to the Business Development manager, your clinical leadership will be crucial in planning, implementation, evaluation and monitoring activities related to home care service delivery. You will collaborate with both internal and external partners to help shape business cases that bring innovative models of care to life.

In collaboration with key stakeholders, you will address quality, safety and risk management issues, including the development of improvement plans. Clinical work in the field will allow you to validate new workflows that directly impact clients and their caregivers.

The ideal candidate will bring experience in mostly adult and geriatric care, complemented by exceptional clinical, leadership and communication skills. Experience in change management or health-system innovation is a strong asset.


 

Key Areas of Accountability:

  • Familiar with the principles, practices, and methods of community nursing and service delivery to patients as well as program development, implementation and evaluation
  • Collaborate with members of the management team to establish recommendations and plans for improvement
  • Provide client and family-centered care independently and in collaboration with other team members to a small roster of homebound patients.
  • Conduct comprehensive assessments, diagnose, treat, and formulate care plans for patients in a home-based setting
  • Provides clinical expertise and consultation services to other members of the care circle.
  • Maintain accurate and timely documentation of patient care and progress.
  • Design and deliver learning sessions and job aids for frontline staff


Required Qualification:

  • Currently registered with the CNO as a Registered Nurse in the Extended Class.
  • Current RNAO or NPAO membership with LAP or equivalent.
  • Strong clinical skills with the ability to work autonomously and within a team.
  • Recent experience in community nursing, geriatrics, and chronic disease management an asset.
  • Excellent communication and interpersonal skills, with a compassionate approach to patient care.
  • Ability to manage time effectively and adapt to changing environments.
  • Proficient in electronic medical record (EMR) systems and technology used in home healthcare.
  • Commitment to continuing education and staying current with best practices in geriatric healthcare.
  • Valid driver’s license and use of a car on the job is required.
  • Post-graduate training or certification in quality improvement, project management or change management is preferred
