Summary:

 

Reporting to the Program Director of Oncology at Trillium Health Partners (THP), the CNS Nurse Practitioner (NP) will join the inter-professional team providing care to patients with primary brain tumors (PBT). This team will improve access to care, decrease fragmentation of care and coordinate services, thereby enhancing a positive patient experience closer to home for this challenging patient population. As a nursing leader, this new role will be involved in the five domains of practice including clinical practice, leadership, collaboration, research and consultation.
 

Responsibilities:

  • In partnership with Physician and administrative leadership, continue to develop the CNS program at THP for PBT patients including the care delivery model, policy and practice standards, protocols and guidelines

  • Conduct comprehensive assessments for PBT patients; formulate a comprehensive evidence-based plan of care in collaboration with the with neuro-oncologists and radiation oncologists; provide treatment and counseling as appropriate

  • Provide care to PBT outpatients receiving chemotherapy and radiation therapy as well as follow up care to PBT patients who have completed treatment (in person and virtual care).

  • Collaborate for care of PBT patients with primary brain tumours when admitted to an inpatient unit.

  • Refer to/liaise with other community services such as palliative care as appropriate

  • Develop appropriate evaluation of the program including CCO performance metrics to monitor quality and best practice

  • Utilize evidence-informed guidelines to guide the development of quality-of-care improvement strategies in the trajectory of patient care.

  • Inform the development of key clinical pathways to improve the quality of care, reduce risks, increase patient satisfaction and increase the efficiency in the use of resources


 

  • Design key CNS program elements so that it is positioned strategically within the oncology program at THP, regionally and provincially

  • Further develop key partnerships with other regional cancer programs and CCO.

  • Provide leadership and assist with professional development of team members through mentoring and participating in formal and informal education initiatives.

  • Identify quality initiatives that will strengthen andfurther develop nursing professional practice.

Key Qualifications

  • Minimum masters level nurse practitioner preparation

  • Current registration in the Extended Class with College of Nurses of Ontario (Successful completion of Ontario NP-Adult Extended Class examination)

  • 3-5 years clinical experience in care of the acute care adult patients

  • Clinical experience with this patient population is preferred

  • Experience with program management and program development is preferred

  • Highly developed analytical,problem solving and critical thinking skills

  • Must be able to practice independently and interdependently

  • Excellent communication and interpersonal skills, with the ability to work collaboratively with physicians, inter-

professional team members and community partners including Cancer Care Ontario (CCO)

  • Well-developed communication, negotiation, and conflict resolutions skills that foster the ability to demonstrate leadership in planning, implementing, and evaluation of interventions

  • Excellent oral, written, and presentation skills, with demonstrated competency in information and communication technology

  • Current Basic Cardiac Life Support certification

  • Satisfactory performance and attendance
