Position Summary

The Centre for Complex Diabetes Care’s (CCDC), primary function is to address the needs of
patients living with diabetes and medical complexities, facilitating effective self – management.
The CCDC utilizes an interprofessional collaborative practice model in which the Nurse
Practitioner (NP) plays an integral team member role in assisting patients to self-manage
diabetes complications, severe hypoglycemia/hyperglycemia, or other barriers that challenge
self-management.

Nurse Practitioner (NP), Centre for Complex Diabetes Care (CCDC):
You will demonstrate the competencies to autonomously diagnose order and interpret diagnostic tests,
prescribe and dispense pharmaceuticals and perform procedures within their legislated scope of practice.
You will demonstrate the following competencies established by the Canadian Nurses Association Canadian
Nurse Practitioner Core Competency Framework (May 2010): Clinical Practice, Collaboration, Consultation
and Referral, Research, and Leadership.
Driven by a fundamental passion and commitment to Patient/Family Centered Care and Inter-professional
Practice, you will provide clinical leadership in the assessment of patients and the development,
implementation and evaluation of individualized care plans based on advanced practice within the clinical
specialty. You will function in an advanced practice role in managing complex diabetes in the presence of
acute/chronic illness and promoting wellness. You will also develop strong internal and external networks
of consulting partners to ensure the needs of patients are met through the CCDC and in collaboration with
primary care practitioners and specialists in the community. You are great relationship builder, and foster
close contacts with peers across the entire organization. You will take on the shared responsibility for
ensuring that health and safety policies are followed to provide for a healthy environment for patients,
staff, physicians, volunteers and families.

On a practical level, you will

• Perform a comprehensive diabetes physical and psychological assessment, develop, implement, and evaluate a plan of care to address identified needs of patients in the CCDC
  
• Influence clinical practice and patient outcomes and collaborate with the broader health care team, the Primary Care Physician, Endocrinologist, other Specialists, the Diabetes Education Centre and community partners regarding complex and challenging patient care situations
  
• Create, utilize, and/or disseminate new nursing knowledge, mentoring students and participate in formal and informal staff education
  
• Provide pharmacological interventions that include: Best Possible Medication History, prescribe and dispense medications as per RN (EC) practice standards and implement other medical directives
  
• Provide non-pharmacological interventions and liaises with the clinical programs as well as other community resources to support this population with acute and chronic disease management
  
• Provide Point of Care Testing (POCT) for capillary glucose sampling and/or glycated Hb A1C per medical directive authorization
  
• Initiate and participate in clinical research and facilitate application of NYGH Professional
  Competencies

Qualifications

• Current registration from the College of Nurses of Ontario as an RN (EC)
  
• Current completion of a ‘Prescribing Narcotics and Controlled Substances’ course approved by the CNO
  
• Membership in the Registered Nurses Association of Ontario and/or Nurse Practitioner Association
  
• Masters in Nursing, or in a health related field (e.g. MH Sc), and completion of a Nurse Practitioner Program
  
• Minimum 5 years nursing experience, including 3 years’ experience as an NP in chronic disease management with a diabetes focus.
  
• Certified Diabetes Educator Certification preferred and/or willingness to obtain within 1-2 years of employment
  
• Experience in nursing research, program planning and evaluation methodologies
  
• Cross appointment or eligible for cross appointment with an academic facility
  
• Knowledge of Regulated Health Professions Act (RHPA) as it relates to Legislated scope of inter-professional practices
  
• Expert clinical skills in the assessment and management of complex diabetes patients
  
• Evidence of program design, resource development, change management and report writing skills
  
• Bilingualism in French/English an asset
