The Centre for Complex Diabetes Care’s (CCDC), primary function is to address the needs of
patients living with diabetes and medical complexities, facilitating effective self – management.
The CCDC utilizes an interprofessional collaborative practice model in which the Nurse
Practitioner (NP) plays an integral team member role in assisting patients to self-manage
diabetes complications, severe hypoglycemia/hyperglycemia, or other barriers that challenge
self-management.
Nurse Practitioner (NP), Centre for Complex Diabetes Care (CCDC):
You will demonstrate the competencies to autonomously diagnose order and interpret diagnostic tests,
prescribe and dispense pharmaceuticals and perform procedures within their legislated scope of practice.
You will demonstrate the following competencies established by the Canadian Nurses Association Canadian
Nurse Practitioner Core Competency Framework (May 2010): Clinical Practice, Collaboration, Consultation
and Referral, Research, and Leadership.
Driven by a fundamental passion and commitment to Patient/Family Centered Care and Inter-professional
Practice, you will provide clinical leadership in the assessment of patients and the development,
implementation and evaluation of individualized care plans based on advanced practice within the clinical
specialty. You will function in an advanced practice role in managing complex diabetes in the presence of
acute/chronic illness and promoting wellness. You will also develop strong internal and external networks
of consulting partners to ensure the needs of patients are met through the CCDC and in collaboration with
primary care practitioners and specialists in the community. You are great relationship builder, and foster
close contacts with peers across the entire organization. You will take on the shared responsibility for
ensuring that health and safety policies are followed to provide for a healthy environment for patients,
staff, physicians, volunteers and families.
