Nurse Practitioner – Long-Term Care (West Toronto)
We’re partnering with a respected long-term care facility in West Toronto to find a passionate and experienced Nurse Practitioner (NP) to join their interprofessional team. Position Overview:
This is a full-time, on-site, independent contractor role. Part-time availability will also be considered. The position is divided into 80% direct resident care and 20% research, education, and leadership.
Key Responsibilities:
- Deliver comprehensive primary health care to long-term care residents
- Lead clinical decision-making around admissions, discharges, palliative care, dementia, responsive behaviours, and end-of-life care
- Conduct detailed health histories and physical assessments; order and interpret diagnostic investigations
- Collaborate with residents, families, and staff to develop person-centred care plans
- Apply critical inquiry and clinical reasoning to diagnoses and interventions
- Select and implement both pharmacological and non-pharmacological treatment plans
- Ensure safe and effective care delivery; identify and mitigate risks
- Coordinate discharge planning and continuity of care across services
- Contribute to and lead quality improvement initiatives and staff education
- Participate in research aligned with resident wellbeing and collaborate with external partners
Qualifications:
- Registration with the College of Nurses of Ontario (CNO) in the Extended Class
- BScN required; Master’s in Nursing (MScN or MN) an asset
- Minimum 3 years’ experience as a Nurse Practitioner in Canada
- Long-term care, geriatrics, palliative, or dementia care experience strongly preferred
- Demonstrated leadership and teaching capabilities
- Professional liability insurance (e.g., RNAO or equivalent)
- National certification in gerontology is an asset
- Comfortable working with diverse, complex resident populations
Additional Requirements:
- Available to work on-site in Toronto (not remote)
- Flexibility for occasional evening/weekend/on-call coverage
- Strong interpersonal skills, calm demeanour, and ability to handle complex care needs
- Familiarity with RNAO Best Practice Guidelines and MLTC Regulations and Act
