One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.
We are currently seeking a 1.0 FTE Nurse Practitioner to join our team of highly skilled nurses, system navigators, social worker, dietitian, nutrition worker and recreation therapist in a team model, supporting the delivery of services through our Community Wellness Hubs. Community Wellness Hubs provide barrier free access and attachment for clinical care, illness prevention, and health promotion services in community settings while fostering strong partnerships across the Thunder Bay service area.
The Nurse Practitioner will provide comprehensive primary health care services with a strong emphasis on disease prevention, health promotion, client education, and empowerment. This role works collaboratively with internal teams and external partners to deliver mobile outreach and clinic-based services within social housing units and other designated community locations.
The position supports individuals who face barriers to accessing care, including those who are under-housed, living with mental health challenges or substance use concerns, managing complex health conditions, members of the 2SLGBTQIA+ community, newcomers, and individuals without insurance coverage.
The position has a competitive salary, the Health Care of Ontario Pension Plan and an excellent group health benefits package.
The salary range for this position is from $108,930 to $130,286 per year. We currently do not use AI to screen resumes.

QUALIFICATIONS:
BScN from a recognized university.
Current Certificate of Registration as a Nurse Practitioner (Extended Class) with the College of Nurses of Ontario (CNO).
Demonstrated proficiency in comprehensive nursing assessment, physical examination, diagnostic reasoning, and treatment within the RN(EC) scope of practice.
Ability to utilize technology to support virtual care, education, and team collaboration.
Experience supporting individuals living with mental health and addiction challenges.
Strong awareness of and sensitivity to socio-economic, cultural, and systemic barriers impacting health.
Demonstrated understanding of health equity and experience working with populations who face barriers to care (under-housed, mental health and substance use concerns, complex health histories, 2SLGBTQIA+, newcomers, non-insured).
Commitment to community-based, client-centred primary health care.
Excellent communication and interpersonal skills, with the ability to work effectively both independently and within a multidisciplinary team.
Flexibility in scheduling and ability to work across multiple community sites.
Awareness of northern and rural health care needs.
Valid driver’s license, access to a reliable vehicle, and ability to travel between service locations.
