Job Summary:
Under the Sioux Lookout Area Primary Care Team (SLAPCT) and within the scope of practice of a Nurse Practitioner (NP), the NP will be responsible for providing primary health care to clients within the Sioux Lookout area. The NP assesses and diagnoses client health for acute illnesses and supports clients to manage chronic illnesses. The NP works with the client and the community to promote healthy lifestyles and disease prevention. The NP collaborates and consults with members of an interdisciplinary team to address issues experienced by clients and within the community. The NP will provide holistic and integrated care to all ages within the Sioux Lookout catchment area, including the 33 First Nations communities.
The NP will:
- Provide holistic care to individual clients or groups of clients including family members and significant others.
- Initiate programs in response to needs and new and existing

The Nurse Practitioner will work out of the Primary Care Team department and will be directly accountable to the Nurse Manager.
Salary Range: $116,144 – $169,608 per annum

Qualifications
- Persons of First Nations ancestry will be given preference (OHRC, Part II, Special Employment).
- Current registration as a Registered Nurse in the extended class, with the College of Nurses of Ontario.
- Current liability insurance.
- New graduates are welcome to apply, however preference will be given to those with experience working as a Nurse Practitioner in a clinical setting.
- Experience working with all age groups (newborn, children and youth, adults).
- Awareness of and sensitivity to the health issues of a diverse community and experience working with First Nations communities.
- Demonstrated ability to work effectively in a multi-disciplinary
- Ability to provide coaching, mentorship, and guidance to community-based health
- Excellent relationship-building skills to initiate and maintain working relationships with senior management, and internal and external
- Excellent verbal and written communication
- Experience with electronic medical records.
- The ability to function effectively during periods of rapid change and transition.
- Knowledge of or willingness to learn about communicable diseases specific to the population we serve.
- Ability to communicate in one of the First Nations dialects of the Sioux Lookout region is an asset.
- Ability to perform the requirements of the position on a regular basis.
