SALARY: $32.84– $44.96 Per Hour
SUMMARY:
Under the direction of the Manager, or designate, and in collaboration as a member of a multidisciplinary team, the Licensed Practical Nurse (LPN) works collaboratively with the care team to support residents in establishing and maintaining a pleasant, safe and homelike atmosphere by providing nursing services and interventions to residents based on assessment and care planning procedures. The LPN co-ordinates health services, and provides support in activities of daily living, including administering medication.
The LPN functions as an independent practitioner and accesses the Registered Nurse (RN)/ Registered Psychiatric Nurse (RPN) or Clinical Supervisor for collaborative support and/or clinical guidance in situations that fall outside the scope of an LPN, such as increased resident acuity or care. The LPN operates in accordance with the competency guidelines and scope of practice within the Standards of Practice as outlined by the British Columbia College of Nurses and Midwives, and in accordance with organizational operating policies and standards and department specific protocols.

What You Will Do
Key Responsibilities:
- Provides safe and respectful nursing care to an assigned group of residents, according to established policies and procedures, and in combination with evolving individualized care plans and setting care priorities.
- Conducts head-to-toe assessments, monitors and recognizes abnormal signs or symptoms including laboratory values, and communicates with the RN/RPN if warranted. Consults other members of the interdisciplinary care team as required. Supports and partners with the RN/RPN in the care of residents with complex or unpredictable outcomes.
- Contributes to the development and ongoing changes to the individualized plan of care, including attending and providing input into multidisciplinary care planning meetings and family conferences.
- Performs nursing care and procedures and evaluates outcomes. Administers medications to assigned residents, according to established procedures, and monitors medication’s effects. Maintains communication with RN/RPN or other members of the interdisciplinary team as required.
- Participates in admissions, discharges, and transfers of residents according to organizational standards.
- Assesses resident’s/family’s actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs. Where appropriate, encourages resident participation in activities of daily living.
- Documents and updates all relevant information about care and procedures, such as assessments, status, progress, treatments, medications administered and discharge planning, to achieve resident safety and comfort and in compliance with established procedures and standards.
- Assists with the orientation of new staff and student preceptors by performing duties such as sharing knowledge about the model of care, demonstrating work procedures, proper use/location of equipment, sharing clinical knowledge, using effective communication, and acting as a positive role model.
- Participates in continuous quality improvement and program evaluation activities, education sessions, team conferences, meetings and committees.
What You Bring
QUALIFICATIONS:
Education and Training:
- Graduate of a recognized program for Practical Nurses.
- Registration with the BC College of Nurses and Midwives (BCCNM) as a practicing LPN Registrant.
Skills and Abilities:
- Demonstrated knowledge and competencies to work at full scope of practice as currently required by the BCCNM.
- Communicate effectively, both verbally and in writing, with residents, families, co-workers, members of the multi-disciplinary care team and the public.
- Ability to deal effectively with others.
- Ability to organize work and implement a plan of care.
- Effectively model behaviours associated with the Home for Us philosophy of care and mentor staff in the same.
- Ability to operate related equipment.
- Physical ability to carry out the duties of the position.
