Job Purpose:
Reporting to the Mobile Outreach Street Health (MOSH) or NECHC Primary Care Manager, the Nurse Practitioner provides health care service as a member of the collaborative MOSH team that includes physicians, nurse practitioners, nurses, occupational therapists, health case managers and others to a designated client population group requiring focused health and illness care. The nurse practitioner is licensed in a nurse practitioner class pursuant to the regulations of the Nova Scotia College of Nurses. The practice of a nurse practitioner includes the provision of specialized health services, which include harm reduction, health promotion, illness, and injury prevention, coordination and management of acute and emergent health problems, and education and advocacy relevant to the patient population. Location of work would primarily include a clinical setting at the North End Community Health Centre (NECHC), and other NECHC and community sites which may include homes. Work with MOSH might also include outreach with MOSH Nurses from a mobile clinic or partner location in the community.
