This position is for our Harold & Grace Baker Centre Home, located in Toronto, ON.

What you’ll be doing

  • Performs a person-centered health assessment of residents on admission, annually and as needed (including quarterly medication reviews) to develop a person-centered plan of care while collaborating with resident, family interdisciplinary team and other health professionals
  • Evaluates the effectiveness of interventions and adjusts plan of care as needed in collaboration with resident or SDM, family, and/or interdisciplinary team.
  • Performs appropriate screening and diagnostic investigations, interpreting results and assumes responsibility for follow-up.
  • Accurately documents assessment plan, interventions and evaluations of resident care.
  • Promotes knowledge development of clinical staff by integrating best practices in resident care while providing leadership and involves the interdisciplinary team in quality improvement initiatives.

What you bring

  • College of Nurses of Ontario registration in the Extended Class
  • Baccalaureate in Nursing (BScN), Masters Degree in Nursing (MScN or MN) an asset
  • Experience in Gerontology/Certification from the Canadian Nurses Association is an asset
  • Membership with Nurse Practitioners’ Association of Ontario (NPAO) (preferred)
  • Acquired professional liability insurance (with either RNAO or other liability insurance) - National certification in gerontology an asset
  • Knowledge about primary health care issues in long-term care facilities.
  • Must provide a complete and current (within three months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one.
