Job Summary



This position works as part of an interdisciplinary primary health care team.  Under the direction of the Primary Health Care Manager, the Nurse Practitioner will provide  primary health care services to the community. The Nurse Practitioner will be responsible for assessment, planning, implementation and evaluation of care for clients in the community both independently and collaboratively with a physician, depending upon the complexity of client needs.  This includes: health promotion and protection; first line treatment and emergency services; provision of supportive counselling and instruction in management of medical conditions; disease and injury prevention; initiating and conducting community needs assessments and participating in evaluative projects; and collaboration with and referral to other health care professionals.  This is a community-based position therefore the incumbent is required to work at a high level of independent clinical practice.


 


This position will be part of the Intermediate Care Service within the Primary Health Care Networks.  This service addresses a provincial health system service gap, where some seniors (adults aged 65+) with multiple complex health issues have difficulty in accessing an office based primary care provider.  The goals for this service are: to provide the most at risk seniors with home based access to higher level health care providers, including MDs and NPs; to mitigate the risk for worsening health states and acute care episodes leading to ED visits and hospitalizations for at risk seniors; to reduce system pressures on acute care services, particularly EDs and hospitals; to provide cost effective and high quality health care using a patient centered and community based approach. Practitioners in this service are assigned to one of the Primary Health Care Networks and provide support to the interdisciplinary Primary Health Care team in client care provision not exclusive to the seniors' population.

Education


Graduate from an approved School of Nursing.  Baccalaureate Degree in Nursing is desired.  Certification in Primary Care Nurse Practitioner or equivalent program is required.  Possession of, and ability to maintain, current registration as RN (NP) with the College of Registered Nurses of Saskatchewan (CRNS).  Current BCLS certification.


Experience


Requires a minimum of three (3) years experience as a Registered Nurse in primary care, public health or acute care.  Requires a minimum of one (1) year recent experience in community needs assessment, community development and health promotion.

  • Must possess a valid Saskatchewan driver's license.  Some assignments may require access to a vehicle for work purposes.

  • This is a community based position providing service in clinic and Primary Healthcare sites. Travel required to various community sites including rural within Primary Health Care networks (Regina). Shifts will be assigned to one of the Primary Health Care Networks based on operational needs.  Home base for this position is be 2110 Hamilton Street
