Duties and Responsibilities

 

Nurse Practitioners will be placed in a Long-Term Care Home or hospital setting. The specific responsibilities and duties vary slightly between locations, as well as from shift to shift. These expectations will be communicated during the site-specific orientation. Duties and responsibilities may include, but are not limited to:

 

  • Take direction from the Manager/Supervisors and be the primary care provider to LTC residents.
  • Contribute to the continuity of care.
  • Assess, diagnose, and prescribe treatment with emphasis on health promotion and disease prevention.
  • Coordinate and deliver primary care services, embed quality improvement, and lead consultation and engagement with healthcare staff and partners.
  • Respond to emergency situations and implement appropriate interventions.
  • Document care and progress by preparing progress notes, written treatment plans and clinical summaries.
  • Collaborate with an inter-professional team in the development of the patient plan of care.
  • Advocate for and encourage patient participation in their plan of care.
  • Support patients in the implementation and modification of their treatment plan.
  • Creation and implementation of quality improvement plans and supporting the implementation of evidence-based practices.
  • 70% focus on direct resident care and 30% focus on education, quality improvement, leadership and research.
  • Interpret and apply legislation to practice (i.e., Healthcare Consent Act, Personal Health Information Protection Act, Regulated Health Professions Act, etc.).
  • Adhere to the Occupational Health and Safety regulations set by the province, as well as the policies and procedures of Superior Staff it and any partnered homes.
  • Practice in accordance with College of Nurses of Ontario Practice Standards and Ministry Expectations.

  • Proficient in technology and have experience managing electronic health records.

 

Qualifications

 

  • Completion of Masters of Science in Nursing (MScN) (required).
  • Current Certificate of Registration with the CNO – Entitled to practice with no restrictions (required).
  • Successful completion of the CNO Adult or Primary Care Nurse Practitioner examinations (required)
  • Minimum 3 years’ experience in a Nurse Practitioner Role (required).
  • LTC experience is considered an asset.
  • Satisfactory Criminal Record Check including Vulnerable Sector Search.
  • Proof of full COVID-19 vaccination (preferred).
  • Must have access to own transportation.
  • A valid driver’s license, access to own vehicle and possessing own car insurance are considered an asset.

 

**ALL NECESSARY PPE WILL BE PROVIDED ON SITE**

 

Salary: $82.50/hr
