Woolwich Community Health Centre (WCHC), a multi-site health centre with locations in St. Jacobs, Wellesley, and Linwood requires a Nurse Practitioner (15 hours per week, 0.4 FTE Permanent) and an additional (22.5 days per week 0.6 FTE contract) in general primary care which will go until end of May 2025 with possibility of extension.


The NP will work within an inter-disciplinary team to provide comprehensive primary health care to a registered client case load and also within the Community Diabetes Program. The Community Diabetes Program (CDP), Waterloo Region is an innovative community-based program. The program provides insulin starts and adjustments, ongoing follow up as required for people who are diagnosed with Type 2 diabetes and those with existing Type 2 diabetes, group and individual education. Woolwich Community Health Centre is responsible for program and service delivery to clients who are residents of Woolwich, Wellesley and Wilmot Townships and North Waterloo.


Required Skills and experience:

  1. BScN with Primary Health Care Nurse Practitioner certificate and registered with The College of Nurses of Ontario in Extended Class.
  2. Certified Diabetes Educator certificate, or experience and willing to complete CDE credential.
  3. Broad-based nursing experience with 2-5 years in primary health care setting an asset.
  4. Commitment to working as part of an inter-disciplinary team that includes registered dietitians, chiropodist, nurses, counsellors, physiotherapy and community health team staff.
  5. Skills/experience to fulfil major functions of role: wellness care; assessment, diagnosis and management of episodic illness; managing chronic disease; and group work.
  6. Strong communication, organizational, and issue management skills.
  7. Excellent interpersonal and problem-solving skills, within the inter-disciplinary team setting and with clients and community members.
  8. Flexibility to work one evening per month
  9. Knowledge of/experience with health issues across the lifecycle. Specific expertise in the following areas would be considered an asset: geriatrics, palliative care, mental health, chronic disease management, maternal and infant/child health as well as familiarity with working in a rural setting.
  10. Strong facility with computer; previous experience with electronic medical records would be an asset.

Compensation & Benefits:

  • Salary of $101,905 to $122,178 per year (pro-rated to working status) ($52 to $63 per hour)
