We are looking for a self-directed, independent Licensed Practical Nurse with strong interpersonal skills to join our Home & Community Care team. About the facility: LLRIBHS promotes health in our communities for the wellbeing of all members. We provide accessible, high quality health services and programs. The Home & Community Care program is based out of the Jeannie Bird Clinic. This position will provide community-based home care nursing services to members living in La Ronge, including three (3) surrounding communities of LLRIB.

 

About the position: The Licensed Practical Nurse must be able to work within their scope of practice to provide nursing care, treatments and procedures. The LPN will also be responsible for evaluating client progress, and updating the clients care plan based on nursing assessment.


Duties:

  • Providing nursing care for home care clients as required;
  • Perform Nursing Treatments and Procedures, as ordered by a physician;
  • Evaluating client progress and updating care plan on an on-going nursing assessment;
  • Providing palliative nursing care and coordinating in-home respite services;
  • Provide case management to coordinate multiple services, ensuring client is receiving safe, effective and appropriate care;
  • Manage client information through an electronic charting system

 

 

Qualifications:

  • Registered and in good standing with the Saskatchewan Association of Licensed Practical Nurses (SALPN), or eligible to apply;
  • Acute or home care experience is an asset;
  • Must be able to work independently, be self-directed and demonstrate organizational skills;
  • Proficient computer skills and experience using Microsoft Office;
  • Excellent verbal and written communication skills;
  • Excellent problem solving & organizational skills;
  • Excellent interpersonal, & public relations skills;
  • Ability to speak/understand Cree is a definite asset;
  • Must possess a valid Class 5 Driver’s License;
