Southlake is excited to announce new opportunities in our innovative Extensivist Clinic, opening in early 2025. A first-of-its-kind, intensive outpatient care program designed to deliver leading-edge, patient-centered care. This Clinic is a collaboration of a dedicated inter-disciplinary team, including Nurse Practitioners, RNs, RPNs, Social Workers, Pharmacists, and others, to ensure comprehensive patient care.
The Extensivist Clinic will provide integrated care for patients to prevent unnecessary admissions and ensure safe, earlier discharges with coordinated follow-up. It provides a broader approach to sustaining patient health and stability over time.
Building on lessons learned from the successful Southlake@home program which has now spread across the province, this Clinic serves as a critical bridge between inpatient and outpatient care and community services. It offers stabilization and ongoing follow-up for patients Located on our hospital campus in Newmarket, the Clinic takes advantage of Southlake’s robust resources and expertise to enhance patient outcomes.

JOB SUMMARY:
The Registered Nurse (RN) role models clinical excellence and the core competencies of the practice standards of the College of Nurses of Ontario (CNO), as well as the policies and procedures of Southlake Regional Health Centre (Southlake). The RN works collaboratively as a member of the health care team. The RN provides direct patient care to patients based on the standard outlined in the College of Nurses of Ontario. This standard has an impact on decision making related to care-provider assignment as well as the need for consultation and collaboration among care providers. All nurses make a commitment to lifelong learning.
The Registered Nurse (RN) is responsible for coordinating and delivering comprehensive care to vulnerable populations. The RN will work closely with patients, their families, medical providers and the inter-disciplinary team to manage, monitor, and optimize the care of these patients, ensuring a high quality of life and preventing complications.
• Educate patients on the importance of following prescribed medication regimens, addressing potential side effects, and providing guidance for missed doses to support adherence.
• Monitor symptoms and overall health to detect early warning signs of deterioration or complications, taking preventive measures to avoid hospitalizations or worsening conditions.
• Collaborate with the Primary Care Provider (PCP), specialist, and other inter-professional team members to create and adjust individualized care plans based on patient progress and evolving health needs.
• Equip patients and their families with the knowledge and resources needed for effective self-management of chronic conditions through education and ongoing support.
• Coordinate referrals and facilitate communication with specialists, incorporating their recommendations into comprehensive care plans to ensure continuity of care.
• Maintain awareness of external programs and resources, referring patients as needed to provide additional support and services beyond the extensivist clinic.
• Facilitate smooth transitions between different levels of care, such as hospital to home or primary to specialized care, ensuring all aspects of the patient's health and well-being are addressed.
• Act as the primary point of contact for the designated patient population, managing urgent concerns, coordinating timely interventions, and prioritizing the care of high-risk or complex patients.
• Collaborate with the extensivist care team to deliver seamless patient care and document all interactions and services provided in the electronic medical record (EMR) system.
• Active registration and good standing with the College of Nurses of Ontario (CNO).
• Bachelor of Science in Nursing (BScN) from an accredited institution is preferred.
• Additional certification in chronic disease management, case management, or a related field is preferred.
• Current member of Registered Nurse Association of Ontario (RNAO)
• Current BCLS certificate required
• Current ACLS certificate preferred
• Proficient in using electronic medical records (EMR) systems and other healthcare technology commonly used in Ontario’s primary care settings.
• Strong knowledge of internal medicine and preventative practices, integrated care models, and multidisciplinary team coordination.
• Demonstrated excellent interpersonal, written and oral communication skills.
• Understanding of and sensitivity to the diverse cultural, social, and economic backgrounds of Ontario's patient population, ensuring equitable and accessible care.
• Commitment to ongoing professional development and staying current with best practices in chronic disease management and care coordination.
• Ability to prioritize, manage time effectively and be flexible in a very active work environment
