What We Need:
As a member of the Adult Diabetes Clinic team, you will provide diabetes education to our patients in the outpatient clinics in both group and individual settings. In this role, you will, in collaboration with the interdisciplinary health care team, be responsible for comprehensive assessment, care planning and education for patients and families with Diabetes. You will practice in an environment where the emphasis is on professional practice and customer education, empowerment and encouragement;
You will co-ordinate your services with the other staff in the Clinic, which may require day and evenings as well as potential travel to our satellite sites. In addition, you will participate in ongoing quality improvement and promotional activities, committees and the development of standards and policies as it relates to our Diabetes Program.
Your strong counselling skills coupled with well-developed analytical, problem-solving and interpersonal skills will assist you in the provision of quality patient care.

The hours of work are: 8 or 12 hour shifts over the Monday to Friday period. Please note schedules vary to ensure adequate coverage during the week
