We are presently seeking an enthusiastic individual to join our high functioning interprofessional team as a Nurse Practitioner with a specialized focus on our geriatric patient population. We are taking an exciting and innovative approach with our virtual care platform and are looking for someone with the drive and energy to help promote further innovation and improve client care. Evening and overnight call coverage will be required.


Responsibilities:


  • Perform advanced and comprehensive health assessments
  • Use evidence-based practice to formulate diagnoses and establish comprehensive, holistic, therapeutic care plans.
  • Technical skill utilizing virtual care software and an open mind to fast-paced improvements driven by our technical team
  • Provide clinical care through a virtual hub and obtain consent for treatment
  • Provide diagnosis of illness and perform interventions based on the NP scope of practice
  • Order and interpret diagnostics tests and prescribe medications
  • Manage disease specific pain and symptoms
  • Follow up with clients in which the primary appointment may have been initiated by another NP or MD.
  • Identify, assess and meet the educational needs of clients, their families and other informal caregivers
  • Provide client education, coaching and counselling related to both acute and chronic conditions
  • Participate in regular business meetings to assist in program development and ongoing monitoring and evaluation
  • Be available for on-call and after office hour services for Rhynocare clients
  • Provide leadership and role modeling in critical thinking, problem-solving, ethical decision-making and use of evidence to inform service planning and system design.

Qualifications:


  • Completion of a recognized Nurse Practitioner program
  • Primary Health Care Nurse Practitioner preferred
  • Current registration with the College of Nurses of Ontario in the Extended Class
  • Minimum 3 years nursing experience, including 2 years experience as an NP
  • BLS-HCP
  • Ability to speak a second language is an asset
  • Have access to a home office
