Hours of Work: 7.75 hours per shift, 31 hours per week


Shift Pattern: Days.


Rate of Pay: $62.74/hr - $72.00/hr

This is a flexible part time position. Consideration may also be given to candidates seeking to work 0.4 FTE or 0.6 FTE (15.5 or 23.25 hours/week).

Position Summary


Reporting to the Manager, Medical Clinic & Patient Access, the Nurse Practitioner is responsible for the provision of health services to manage the needs of individuals, families, groups, and communities. The Nurse Practitioner uses the principles of primary health care to guide clinical practice and thus contributes to the models of health care established by Mosaic Primary Care Network.


The Nurse Practitioner uses their in-depth advanced practice knowledge to provide comprehensive assessments, to order and interpret diagnostic and screening tests, to diagnose illness and disease, and to therapeutically manage health concerns, including the prescription of medications and therapies. The Nurse Practitioner works autonomously, as well as collaborate with the team of health professionals in order to provide comprehensive health services for their clients.


This position works in Low-Risk Maternity Group (LMG), Mosaic Medical Clinic (MMC), ACCESS clinic and the Mosaic Pain Management Clinic (PMC). When working at the Pain Management Clinic (PMC), Nurse Practitioners will collaborate with other members of the PMC multidisciplinary team of health professionals to provide detailed assessment and comprehensive health services for patients suffering from chronic pain.


Key Responsibilities


Health Assessment and Diagnosis



  • Performs an advanced comprehensive and focused health assessment.

  • Synthesizes and analyzes health assessment information using critical thinking and clinical reasoning skills.

  • Combines patient assessment findings with scientific and clinical knowledge to make a diagnosis within the nurse practitioner's scope of practice.

  • Order appropriate screening and diagnostic investigations and interpret these investigations based on sound clinical reasoning, scientific evidence, and critical thinking.

  • Appropriately consults with and refers to physicians or other members of the health care team when client needs are beyond the scope of the nurse practitioner, or outside of the individual’s NP’s level of competence.


Health-Care Management and Therapeutic Intervention



  • Collaborates with patients, families, and other members of the health care team to share decision making and set priorities for the management of health concerns.

  • Applies knowledge of pharmacology in selecting, prescribing, monitoring, and dispensing drugs within the nurse practitioners’ scope of practice.

  • Manages the treatment of patients diagnosed by the physician or nurse practitioner within their scope of practice.

  • Initiates interventions for the purpose of stabilizing clients in urgent or emergency situations.

  • Educates patients on pain science, biopsychosocial approach to managing chronic pain in alignment with current pain science as well as the shared messaging of the PMC team.


Health Promotion and Prevention of Illness, Injury and Complications



  • Appropriately and intentionally includes health promotion, illness prevention and capacity building in client relationships and responses to health needs.

  • Participates with other healthcare providers and sectors to plan and develop health promotion/illness prevention programs based on client needs, cultural considerations, evidence-based approaches, and available resources (including screening for populations at risk and population-based harm reduction strategies).

  • Creates or advocates for an environment that facilitates clients’ learning and maximizes their participation and control in meeting their own health needs including those clients living with chronic disease or in an unhealthy environment.


 Documentation



  • Documents all phases of patient care in the appropriate Electronic Medical Record (EMR) to maintain accurate and timely records, as well as recording the details of clinical interactions in MPCN evaluation systems.

  • Actively participates in quality improvement activities for the betterment of patients served and services provided by MPCN.


 Professional Role & Development



  • Explains and promotes the role of the nurse practitioner to patients, the public, legislators, policymakers and other healthcare professionals.

  • Understands the changes in scope of practice from that of a Registered Nurse and accepts the increased responsibilities and accountability of the Nurse Practitioner role including practicing in accordance with legislative acts, regulations and bylaws relevant to the Nurse Practitioner’s area of practice.

  • Understands and incorporates the additional professional and legal standards and ethical decision-making involved in the diagnosis and treatment of acute and chronic illnesses, including prescribing medication.

  • Identifies and works to achieve own professional development needs which support goals set in performance reviews.

  • Other responsibilities as required.

    Qualifications 


    Education & Licenses



    • Graduate from an accredited Nursing Program.

    • Master’s degree in Nursing required.

    • Nurse Practitioner Certificate in the Family/All ages designation.

    • Active registration as a Nurse Practitioner with the College and Association of Registered Nurses of Alberta (CARNA).

    • Current Neonatal Resuscitation Program.

    • Current BCLS (Basic Cardiac Life Support for Health Care Providers).




         Experience



        • Minimum 3 to 5 years’ experience in an acute care and high-volume setting required.

        • Minimum of 3 to 5 years as a Nurse Practitioner in acute care or community setting with preference for Women’s Health and Chronic Disease experience.

        • Experience in culturally diverse and vulnerable populations preferred.


        Knowledge, Skills & Abilities



        • Knowledge of professional and legal components of nurse practitioner practice.

        • Demonstration of strong clinical and technical competencies including advanced physical assessment, diagnostic reasoning, and therapeutic management of disease.

        • Ability to adapt practice based on appropriateness for the client and within the context of a given situation.

        • Demonstration of understanding in caring for diverse populations with various health, cultural, and socioeconomic backgrounds.

        • Ability to collaborate with other health care professionals in the provision of health care.

        • Ability to plan and implement activities and teaching aimed at the promotion, maintenance and restoration of health, and prevention of illness and disease.

        • Strong communication, teaching, problem-solving and leadership skills.


        Working Conditions



        • Occasional evening and weekend hours required.

        • Ability to travel on public transportation or valid driver's license with access to own car in good repair is required.
