In partnership with NS Health, PRAXES operates the Mentor Clinic: a demonstration clinic for the “health home” model of primary care, which involves a well-supported multi-disciplinary team with the patient as the center point. Relationships are a key element of the health home, and every role has a vital and valued part to play in the success of the team. We believe that every activity that touches or supports a patient is important and should be a positive experience for both the patient and the provider. 


Mentor Clinic is separate from but contracted to NS Health. It is publicly funded – there are no patient fees. Mentor means ‘experienced trusted advisor.’ The broad-based inter-professional and layperson clinic will 1. mentor one another 2. mentor patients 3. patients will mentor us.

Nurse Practitioner Role: 

  

Nurse Practitioners are clinical decision makers who work collaboratively with other health care professionals to support patients who are rostered to the clinic. They provide health promotion, disease prevention and illness management for individuals, families, groups, communities, and populations.  

  

** Note: This position is based in Halifax Nova Scotia and so the successful candidate will need to be physically located in Halifax. ** 

  

Hours of Work: Part time - Saturdays. One 4 to 8 hour shift weekly or casually. Candidates must be available to work on this day. 

  

Compensation: $73.09 hourly, inclusive of vacation pay. 

  

Expectations: 

  

  1. Patient Care

  

  • Examine patients and take their medical histories. 
  • Prescribe medications. 
  • Co-ordinate or manage primary patient care. 
  • Order and interpret laboratory tests and diagnostic procedures. 
  • Make referrals to other team members (RN, PT, Dietitian, OT, SW) and specialists as required. 
  • Advise patients and their families on health care including health education, promotion, disease, illness and accident prevention. 
  • Perform a patient advocacy role. 
  • Provide primary care in person or remotely (via telephone or video). 
  • Provide care for same-day, urgent appointments  
  • Answer phone calls from the lab or from pharmacies; 
  • Respond to urgent tasks/lab results as required during the shift. 

  

Skills and Qualifications: 

Must-have 

  • Master in Nursing with Nurse Practitioner Certification. 
  • Nova Scotia NP License (in good standing) and Medical Liability Insurance. 
  • Solid Computer Skills and working knowledge of the Telus MedAccess EMR system and Microsoft office suite. 
  • Ability to work collaboratively with other healthcare providers and administrative staff. 

Nice-to-have 

  

  • Experience in a multi-disciplinary primary care environment 
  • Experience with urgent, same day presentations
