Status: Contract Full Time; 14 month duration 

Salary Range: $125,186-$156,484 per annum plus comprehensive benefits

Location: 7075 Rexwood Road, Mississauga 

Hours of work: 35 hours per week

As a Nurse Practitioner, you'll be at the heart of improving the lives of residents in long-term care. You’ll work closely with an amazing interdisciplinary team, including the Administrator and the Practice, Innovation, and Education team, to ensure top-quality care and outcomes for those with medically complex needs. This is your chance to shine as a key leader, share your specialized knowledge, and collaborate to enhance healthcare delivery every day.

 

What You’ll Do:

  • Lead with Compassion: Guide our dedicated Registered Staff in delivering the best clinical care. You'll perform in-depth health assessments, develop comprehensive care plans, and work closely with physicians, families, and decision-makers to treat both acute and chronic health conditions.
  • Build Meaningful Relationships: It’s not just about clinical expertise—it’s about people. You’ll mentor and coach staff while building strong, compassionate relationships with residents and their families. Together, you’ll create a culture of healing, growth, and comfort.
  • Lead with Purpose: Manage the High Intensity Needs budget, order diagnostic tests, interpret results, and make clinical decisions—all while providing leadership that ensures the highest level of care and supports the Centre’s commitment to quality, holistic care.
  • Create a Culture of Continuous Improvement: Lead by example and inspire collaboration, education, and a focus on excellence. You’ll foster a space where healing isn’t just physical—it’s emotional and holistic.

What You’ll Need:

  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult)
  • At least 5 years of related experience
  • CPR certification for healthcare professionals and standard first aid
  • Vulnerable Sector Check (VSC) completed to Regional standards
  • A combination of education and experience will be considered

Your Skills and Abilities:

  • Advanced expertise in assessing and treating older adults with conditions like delirium, cognitive impairments, and behavioral disturbances
  • Proven ability to support patient and family education, as well as staff training
  • Strong clinical skills in complex disease management and ethical decision-making
  • Computer literacy and attention to detail in documentation
