Job Description:
Position Overview:
Working as a collaborative member of the inter-professional team, the Nurse Practitioner (NP) functions in an expanded role demonstrating a high level of autonomy and expert skill to formulate clinical decisions and appropriately manage acute/chronic illness and wellness promotion for adults and seniors.
Senior’s Health Clinics:
The NP leads the provision of comprehensive geriatric services in the GAIN (Geriatric Assessment and Intervention Network) Clinic. This is an exciting model of Geriatric Outpatient Care that employs interprofessional comprehensive geriatric assessment (CGA) in the management of frail seniors as well as rapid assessments for our post- discharge patients from inpatients and ED. The NP provides direct patient care focusing on health promotion, maximizing patient safety and function to support frail seniors living at home.
Outreach:
With a growing seniors population in the Scarborough community, supporting our seniors at home is a growing priority for SHN. As an expert geriatric NP, you will provide outreach virtual and/or onsite consultation support to our frail senior patient population in LTC homes, retirement homes and in the community (ie home visit as part of GAIN assessment). With a keen ability to obtain critical information provided by care teams in LTC homes, retirement homes and/or in the community, you will assess and provide specialized geriatric consultation to support care plans and necessary intervention.
Consultation Liaison Services:
With growing demands for expert geriatrics services for our inpatient and ED populations, the NP will provide consultation services for frail elderly patients with multiple comorbidities and complex medical regimes in hospital as a member of consultation liaison service. He/she will provide expert knowledge and support to inpatient and ED teams in the care and treatment of frail elderly patients and work with BSO and psychiatry liaison (CL) team as necessary.
This position is ideal for a Nurse Practitioner with a passion for geriatric care. Hours will require flexibility and will include evening and weekend work, and may include home visits. The successful candidate will be required to provide proof of liability coverage.
Requirements:
• Masters of Nursing
• Current registration in the Extended Class with the College of Nurses of Ontario
• 3 years recent clinical work experience with frail older adults as a Nurse Practitioner
• Certification in Geriatric Nursing from Canadian Nursing Association preferred
• Demonstrated ability to exercise independent judgment and to assess situations and problems efficiently and effectively
• Effective verbal and written communication skills
• Strong interpersonal, organizational and problem-solving skills
• Ability to work well within an inter-professional team and to foster effective working relationships
• Ability to exercise initiative and observe strict confidentiality
• Proven good attendance record with capability of maintaining this same standard
• Demonstrates values consistent with SHN’s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation.
• Utilize protective equipment provided and identify and reports hazards
• Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
