Role Summary
Reporting to the Director of Care the Nurse Practitioner is responsible for all aspects of care for residents. This position will work in collaboration with the Clinical Leadership team within our communities, delivering preventive and continuous care towards managing residents' health. The Nurse Practitioner will provide professional treatment autonomously or collaboratively with other healthcare professionals to residents making referrals to specialist physicians and other resources as necessary.
Responsibilities:
- Examine patients and take their medical histories and consult with physicians and other medical practitioners to evaluate patients’ physical and mental health as appropriate.
- Manage chronic health challenges care plans and preventive health initiatives
- Collaborate with the interdisciplinary to develop and implement resident focussed care plans
- Provide education to residents and families as needed on health issues
- Maintain accurate and timely documentation in electronic health charts (Point Click Care)
- Participate in quality improvement initiatives and adhere to professional standards set by BCCNM
- Prescribes drugs within the statutory and regulatory standards, limits, and condition for Nurse Practitioners and within applicable employer policies and procedures.
- Be able to collaborate, consult with and formally refer patients to physicians and other health professionals when appropriate.
- Be able to self-direct, interact, and adapt effectively with other professionals in complex, dynamic situations.
- Adhere to all privacy and confidentiality regulations as required by law.
- Monitors ongoing care, orders appropriate screening diagnostic investigations; interprets reports of investigations and analyzes information to monitor progress and plan treatment.
- Other duties as assigned.
Qualifications and Experience:
- Successful completion of a provisional registration with British Columbia College of Nurses and Midwives (BCCNM).
- Successful completion of the Objective Structural Clinical Examination (OSCE).
- Minimum five (5) years recent nursing experience, preferable in Residential Care and or Home and Community Care.
- Have effective multi-tasking, time management and work prioritization abilities.
- Must demonstrate the ability to communicate effectively and maintain cooperation with all levels of personnel.
- Must be able to effectively build relationships l with residents and their relatives in a manner that demonstrates tact, empathy and patience.
- Computer literacy in electronic health charting Point Click Care
- Must have the temperament necessary to maintain a caring manner while demonstrating a therapeutic approach to the delivery of care.
