The Opportunity:
This dynamic position allows for direct patient care in a fast-paced dermatology clinic. You will provide patients with disease related education, support them with navigating a complex journey in obtaining treatments that require specialized knowledge and connections within the healthcare space that reduce the barriers with accessing these care needs. The successful candidate for this role will take the lead in ensuring all care required before, during and throughout the patient’s journey in accessing specialized medication is seamless to the patient. The nurse will be resourceful and knowledgeable at navigating barriers for patients so that their access to disease altering medicines is effortless, in turn promoting optimal patient satisfaction and outcomes with their care. The Dermatology Licensed Practical Nurse will be the primary point of contact for their assigned healthcare team relating to the care path of patients accessing services that Sentrex offers. The nurse will work as part of the clinic’s multidisciplinary team, ensuring wrap around support for patients internally and externally to the clinic (pharmacy, insurance companies, PSPs).
The ideal candidate is one who is passionate about patient care and working as part of an extended healthcare team that works collaboratively and innovatively to ensure patients get the care they need for optimal health. The candidate will embrace curiosity, ongoing learning, and continues to adapt workflows in order to continuously implement best practices that will create significant change in the way patients receive care.
A Day in the Life (What you will do here):
- Work as a collaborative care team member with HCPs assisting with assessments in an office setting.
- Process data entry activities of pertinent information received at different points of contact during the delivery of services.
- Provide medical support to patients and HCPs, including but not limited to education on specific diseases, therapies, injection training, and disease management.
- Prepare for and attend extensive, specialized training sessions for specific therapies.
- In person and virtual support (including adherence support both inbound and outbound as needed).
- Review patient files prior, during and throughout the patient care journey to ensure optimal care path timelines and safety in delivering care.
- Document and maintain patient records to ensure treatment records are adequately maintained.
- Maintains confidentiality of patient and corporate information and discusses same only with appropriate personnel.
- Liaise with physicians’ offices, pharmacies, services providers, and other parties to provide comprehensive and coordinated service to patients and HCPs.
- Participates in ongoing internal and/or external continuing education activities.
- Administers medications (IM and SC) in the clinic setting as required.
- Monitors patients post injection, and documents as per college standards.
- Hosts patient education based on specific patient needs, individually or in group settings.
- Responsible for scheduling appointments as required for care path adherence.
- Work collaboratively within multidisciplinary health care team providing oversight.
- Proactively offer support and advocacy to patients and HCP which includes but is not limited to enrollment, facilitation of coverage renewals, and changes throughout the patient journey, as well as facilitating reimbursement support to physicians and patients.
- Provide ongoing support on disease state or product related questions.
- Read and review all Canadian product monographs to ensure a comprehensive understanding of each product being prescribed and administered.
- Report and document Adverse Events as per training per Pharmacovigilance requirements.
- Adhere to Sentrex policies and procedures.
- Assist in the development and adherence to forms, policies, and work instructions, to ensure the safe handling, preparation, and delivery of all care.
- Follow and abide by all provincial College of Nurses guidelines while providing patient care. Adhere to professional nursing standards at all times.
- Additional duties as assigned by the manager.
What you need to ensure you are set up for success:
- Minimum 2-4 years of experience of clinical experience (preferred)
- Licensed LPN
- CPR and First-Aid certification required
- Knowledge of Intrahealth EMR an asset
- Able to confidently deliver health teaching, including but not limited to patient self-injection education
- Ability to confidently administer medication IM and or SC.
- Experience with using Excel, Outlook and Word an asset.
- Able to work autonomously with little supervision, seeking information from HCP as required
- Experience in Patient Support Programs is an strong asset
- Experience in Dermatology and Chronic Disease management.
- Understanding of the pharmaceutical industry.
- Strong analytical and mathematical skills.

- Ability to meet deadlines and be flexible with patient and HCP requirements.
