Pharmacy Hours:
Mon-Fri 7:30 a.m. to 5:30

Sat - Sun 7.30 a.m. to 3:30 p.m.

About ACE Team

The Adherence and Community Engagement (ACE) Team is a multidisciplinary outreach team dedicated to engaging hard to reach people experiencing health and social inequity. The ACE Team manages a caseload of 35-40 clients, operating 7 days a week, 365 days a year to provide wrap around health and social supports. We recognize that a client’s health outcomes and their ability to address their basic needs, goes hand in hand. ACE is a dedicated group of pharmacists, nurses, outreach, and social workers who go above and beyond everyday for their clients. Key therapeutic areas include HIV, Hepatitis C, STIs, Wound Care, Chronic Disease Management, Mental Health, and Substance Use Disorders. 

About the Role

The LPN would assess patients and document patient needs and details in the ACE specialized Med Access EMR. LPN would collaborate with other healthcare professionals within the circle of care and often help inform of current medical status and advocate for patient needs. The LPN will meet our clients where they are at and thus spend seven to eight hours in a car with another colleague and one-two hours in the office where they can spend dedicated time finishing tasks.

Key Responsibilities

  • Complete patient intakes and care plans in a timely manner, utilizing tools, forms, and processes as aligned with ACE protocols.
  • Comfort in independently assessing and dosing witnessed medications such as methadone, Suboxone®, and Kadian®
  • Communicate clearly with patients, peers, management, and other healthcare professionals through a variety of media using effective and appropriate communication skills.
  • Coordinate appointments, access to care and social services for clients.
  • Advise the patient to seek further medical attention where appropriate.
  • Answer any questions requiring clinical knowledge, which fall within the scope of practice as a licensed practical nurse in Alberta.
  • Maintain confidentiality of patient information.
  • Provide patient health education, harm reduction, and employ principles of preventative medicine. 
  • Provided clinical care as it relates to physical assessment, minor ailments, medication therapy, and activities of daily living where required and within scope of practice.
  • Follow principles of person-centered care when providing wholistic care.
  • Provide wound care and maintain the cleanliness and stock of supplies and equipment.
  • Ensure accurate and timely completion of appropriate documentation, including client charting in Med Access EMR.
    • Provide leadership in the management of client care.
    • Coordinate and communicate with a multidisciplinary team to demonstrate patient centered care.
    • Be an ambassador for the ACE Team.
    • Demonstrate willingness to learn new skills as required.

 

Education & Qualifications

  • Active registration and valid practice permit with the College of Practical Nurses of Alberta (CLPNA)
  • Graduate of an accredited Licensed Practical Nursing program
  • Active Alberta Class 5 Driver’s License, Clean 3 Year Drivers Abstract
  • Police Information Check with Vulnerable Sector.
  • Certifications (or willingness to obtain): Standard First Aid and CPR.
  • Training in Trauma Informed Care, Cultural Sensitivity, Stigma, and/or Harm Reduction.
  • Certification in Phlebotomy, Advanced Wound Care, Foot Care, and/or Immunizations is an asset.

 

Experience & Skills

  • 1 year experience practicing as an LPN is preferred.
  • Minimum 6 months of experience working with marginalized, high risk and vulnerable populations.
  • Experience administering high risk medications like methadone, Suboxone®, and Kadian®
  • Working knowledge of harm reduction practices, abuse and trauma, sexual exploitation, addictions, mental health, homelessness, inner city, and Indigenous culture.
  • Knowledge of and experience with Motivational Interviewing and using evidence-based models of care.
  • Demonstrated experience de-escalating crisis and potentially violent situations with individuals under the influence of drugs or alcohol, under stress &/or struggling with acute/chronic mental health crises.
  • Demonstrated ability to collaborate with various social and healthcare disciplines to deliver person centered care.
  • Comfortable using mobile technology- smartphones and phone applications, mobile laptops, Google Drive, Google Calendar, electronic platforms for client documentation.
